Formed by the merger of Arden Hill Hospital and Horton Medical Center, Orange Regional is licensed for 383 beds and employs over 2,500 healthcare professionals including over 600 doctors. Orange Regional Medical Center has consolidated two existing campuses onto a single site in Middletown, New York. This completed facility is the first new freestanding hospital built in the State of New York since 1989 and is the largest medical center between the Tappan Zee Bridge and Albany.


    Through a referral process, a Social Worker coordinates discharge planning activities with active involvement of patient, patient representatives, RN Case Managers, Medical Staff, Nursing Staff and other disciplines as required. The Social Worker will ensure that an appropriate and safe discharge plan is implemented upon discharge for patient’s continuum of care. Social Workers are also responsible for identifying a patient’s psychosocial and socioeconomic issues that may interfere with a patient’s continuum of care progression as well as providing supportive counseling and community referrals as necessary, i.e. counseling services, DSS, APS, CPS, homecare.


    Minimum Education/Required Certification:
    MSW with current NYS License. Prior healthcare experienced preferred.

    Minimum Experience:
    A) Must be able to communicate and collaborate/cooperate with other team members, payors and community agencies while maintaining and promoting positive relationships, using diplomacy, tact, assertion and compassion.
    B) Highly developed verbal and written communication skills. Ability to communicate with a wide range of individuals and groups, and work independently. Must have good organizational skills, judgment, safety awareness, problem solving, and time management skills. Requires knowledge of community resources, third party payors, and reviewing agencies.

    Hours: 8:00 AM to 4:00 PM/9 AM to 5:00 PM/10:00 AM to 6:00 PM

    Physical Demands: Minimal physical demands-walking, standing, sitting, daily data entry. Able to manage multiple priorities and fast pace environment.

    Working Conditions: Normal hospital setting. Duties are performed indoors, with walking, standing, sitting and daily computer data entry. Requires flexibility in dealing with rapid activity schedules and time constraints under stressful situations. Minimal physical demands. Direct patient contact per interview and patient physical assessment.
