    Responsibilities

    To assist patients and families to identifr social, emotional, and environmental needs and to connect them with available resources/services through the provision of a full spectrum of discharge planning and social work services in the outpatient setting. Serves as a single source of contact for referring physicians, patients and caregivers to provide resources.

    Qualifications

    • Clinical Social Work license required or must be obtained within one year of hire
    • Breast Navigator Certification required or must be obtained within six months of hire
    • Minimum of three years oncology experience preferred, relative to the patient population
    • Effective communication skills, both written and oral
    • Computer literate; Microsoft office competency required
