    Assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal collaboration outcomes for the oncological patients. Patient navigation in cancer care refers to individual assistance offered to patients, families, and care givers to assist in overcoming barriers and to facilitate timely access to quality medical and psychosocial care from pre-diagnosis through all phases of the cancer journey to survivorship. Works closely with patient, families, physicians, and ACS navigators.


    Education: Graduate from accredited school of nursing.

    Experience: Requires a minimum of three years clinical experience relative to the patient population and scope of care of the department is required: work related experience with specific service line care highly preferred. Effective English communication skills, both written and oral. Ability to use data entry and retrieval system

    License: New York State License for a Registered Professional nurse and
    current registration or valid permit to practice in New York State
    as an RN.
